*Applicants are advised to complete the application form neatly and completely with absolute legibility and appropriateness to avoid rejections. Also, all the required documents must be furnished completely.

*Applicable proof of death can be a Certificate issued from the hospital where death has been occurred or from a Medical Practitioner who analyzed the corpse.

*In case if the death has been occurred in places other than hospitals, a doctor can be called for analysing or any local authority can be approached for a Permission letter.

*Deaths of foreigners occurring inside Nigeria should also be registered in the National Population Commission whereas deaths of Nigerian citizens occurring abroad should be registered with the Nigerian Embassy/Consulate in that country.

*Please note that only one original death certificate will ever be issued from the National Population Commission of Nigeria.
