*Level 1 DDC Passed (Only If application is for Full license for class 1, 2 or 8).
*Level 2 DDC Passed (Only If application is for classes 3, 4 or 5 licenses).
*Provisional License Expired (Only if application is for Full license for class 1, 2 or 8).
