Introduction

The United Health System Card is a document designed to facilitate access to the service network of the Unified Health System (SUS) and contain data about when and where the patient was treated, what services were provided and at what work and what procedures were performed.

Procedure

The card can be obtained in all municipalities in Brazil. Ideally, the citizen can have his/her National Health Card delivered by SUS.

Registrations are done in hospitals, clinics and health centers or locations defined by the local health department, upon presentation of ID, SSN, birth certificate or marriage.

The National Health Card System objectives are to organize and systematize data on the service provided to users, provide the service network of the Unified Health System (SUS) of a tool to facilitate information exchange between various health services, provide information on a person's SUS user anywhere in the country, and generate reliable data and updated to allow planning and exchange of knowledge to support the development and implementation of public health policies.
